Sangamon, Tazewell, and Woodford

We serve individuals with developmental disabilities

in their own homes, nursing homes, day training and

a variety of residential living arrangements. Many

of the individuals have multiple needs including

residential, vocational, educational, medical and

mental health.

Priority of Urgency of Need for Services (PUNS)

Provide entry into statewide database in order to

obtain Medicaid Waiver Funded services

Pre-Admission Screening (PAS)

Evaluating eligibility for Medicaid Funded

developmental disability services

Individual Service and Support Advocacy (ISSA)

Provides independent support and advocacy for individuals

receiving Medicaid Waiver services, such as

CILA, Day Training Only, Home-Based Support

Services and Children’s Waiver

Provides support and advocacy for individuals identified

in the Bogard Consent Decree

Case Coordination

Assessing and linking individuals to available services.

Assistance with school transitions.
